A Feline Muscle-Loss Drug Clears Its First Trial

A Delaware biotech says it ran the first randomized trial of a drug for age-related muscle loss in cats, and posted encouraging results as a preprint. The muscle-height gains beat placebo, but a small sample, an appetite-stimulant confound, and an unpublished safety model mean the real catalyst is the conditional-approval filing coming later this year.

A Feline Muscle-Loss Drug Clears Its First Trial

Age-related muscle loss is one of the quietest ways a cat gets old, and one of the least studied. A Delaware biotech now says it has run the first randomized trial of a therapeutic built to slow that decline, and posted the results as a preprint this week. The numbers look encouraging. They also arrive with enough unanswered questions that operators should read well past the headline.

First feline sarcopenia trial reports muscle gains over placebo

The trial, named PURR-001, tested a once-weekly oral therapeutic against primary age-related sarcopenia, the involuntary loss of muscle that comes with aging rather than with a specific disease. It was randomized, double-blind and placebo-controlled, enrolling 35 elderly sarcopenic cats drawn from both client-owned homes and shelters. The work was posted August 17th, 2026 and is credited to a team at The Cat Health Company alongside collaborators at a Bucharest technical university and two veterinary practices.

Two primary endpoints carried the result. Cats on the drug gained 7.4% in body mass over the study window against 1.1% for placebo, and gained 22.8% in ultrasonographically measured epaxial muscle height against 9% for placebo. The company that ran it, The Cat Health Company, reported that serum biochemistry stayed stable across the trial with no evidence of clinically meaningful treatment-related toxicity. Two cats died during the study against four the authors say would be expected from reference mortality in a cohort this old. The therapeutic is described as a multi-target formulation aimed at conserved aging pathways, and its actives, per the lead author's own public comments, include low-dose rapamycin and the appetite stimulant mirtazapine.

What a 35-cat preprint can and cannot prove

Preprint is the operative word. This is a self-published manuscript on Zenodo, not a peer-reviewed paper and not a regulatory dossier, and the sample is 35 cats. That does not make the data wrong, but it means the result can't carry more weight than a study this size can bear. The best stress test of it happened right in the comments under the announcement, where a commenter pushed back hard and the company's CEO answered every point in public.

The pushback had two parts. First: one of the drug's ingredients is mirtazapine, a well-known appetite stimulant, so how do you know the cats gained muscle and not just fat? Second: one cat in the trial died of pancreatitis following an infection, and rapamycin, the other main ingredient, is linked to pancreatitis and immune suppression at the doses used in human transplant patients. Was that death really unrelated to the drug?

Alex Voda, The Cat Health Company's CEO, answered both in the replies. On the muscle question, he pointed to the ultrasound measurements of actual muscle height, not just body weight, as the number the company is standing behind, since a fattened cat and a muscled cat don't look the same on that scan. On the pancreatitis question, he shared the affected cat's blood work, arguing its white blood cell counts were normal to high, the opposite of what immunosuppression would look like, and that the trial's rapamycin dose was an order of magnitude below the range tied to those transplant-patient side effects. He also noted the study saw fewer deaths overall than a statistical model would predict for cats this old and sick: two, against four expected.

Both are reasonable answers. Neither is settled until the company publishes the full safety data and the mortality math it has promised.

There is also a signal in the placebo arm: those cats gained close to 9% in muscle height on their own, which either points to a selection effect in who got enrolled or to how much supportive care alone moves the needle. For anyone building in feline health, that is the number that complicates a clean marketing story.

The strategic read is that cats are catching up to dogs. When Loyal raised $100M to bring a canine longevity drug toward market, it validated species-specific aging therapeutics as a fundable category, and the feline side had been the neglected half. Muscle maintenance is not a vanity endpoint here either. Cats that hold muscle through chronic kidney disease, the leading killer of senior cats, tend to do better regardless of diet, which is what makes sarcopenia a commercially serious target rather than a wellness footnote. The credibility bar is the same one PetVivo cleared with peer review: in a category thick with unproven supplements, published evidence is the moat, and a preprint is only the first plank of it.

The conditional-approval filing that sets the timeline

The near-term catalyst is regulatory, not scientific. The company has said it expects to submit data for conditional approval in the back half of 2026 and has flagged further news around early September, which likely pairs a data update with a funding or partnership step. That filing, not the preprint, is the event that would move this from LinkedIn to a shelf.

What to watch: whether the peer-reviewed version and the full safety tables hold up the preprint's numbers, whether the promised mortality-model code justifies the reference-cohort claim, and how regulators treat a multi-drug combination whose components are individually familiar but untested together in cats. The Cat Health Company raised a $1.2M round in October 2025 led by Portfolia's longevity fund, which buys a limited runway to convert this into an approved product. Expect the data to keep arriving in public, packaged for owners and investors as much as for veterinarians, because the science and the fundraising narrative are now moving on the same clock.
